In checking the box below I agree that Yoga Spa Lounge 1881, is in no way responsible for the safekeeping of my personal belongings while I attend a yoga class and spa treatments. I understand that classes at Yoga Spa Lounge 1881, may be physically strenuous and I voluntarily participate in them with full knowledge that there is a risk of personal injury, property loss, or death. I agree that neither I, my heirs, assigns, or legal representatives will sue or make any other claims of any kind whatsoever against Yoga Spa Lounge 1881, or its members, staff, or independent contractors for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.

All privates must be cancelled at least 24 hours in advance. Late cancellations will be charged at the full rate.
Yoga Spa Lounge 1881 points do not expire, they will remain on the account until fully redeemed.

I hereby give permission for my participation in yoga classes and spa treatments at Yoga Spa Lounge 1881. I further authorize, without my prior approval, the rendering of any emergency medical treatment that may become necessary due to my participation in yoga.

In consideration of the permission granted to me to participate in yoga classes and spa treatments , I do hereby agree, on my own behalf, to release Yoga Spa Lounge1881, its officials, administrators, employees, representatives and volunteers, from any and all actions, causes of action, damages, claims, or demands of whatever kind or nature which I may have for any injuries, known or unknown, which are incurred by, arise from, or in any way relate to my participation in yoga classes.

I realize that I am participating in yoga classes & spa treatments at my own risk. My signature is binding to this liability waiver this day forth. I have read this release and fully understand its terms. I execute the release voluntarily and with full knowledge of its significance and consequences.
